Fillmore Connections

This is a monthly publication of the Millard Fillmore Elementary School, Moravia Central School District, Moravia, NY. It provides important information about the activities of our students and staff.


FORMS


Miscellaneous Forms

Print each form as needed, complete and return to the address shown on the form or to the school office.

  • Pesticide Notification to Persons in Parental Relation and Staff
    This form is required by law to notify students, staff, and those in parental relationship to students of pesticide applications. The Moravia Central School District does not perform any non-exempt applications. We are still required to provide this notification.

  • Building Use Request
    This is the form to be used by outside groups who wish to make use of school district grounds and facilities. Please read the accompanying Board of Education policy prior to submitting this request.


Elementary School Forms

Print each form as needed, complete and return to the elementary to the school office.

  • Emergency Closing Form
    Use this form to let the school know where your child is to go in case of an early school closing

  • Emergency Information Form
    From time to time emergencies arise, and School personnel must locate a parent of a child, or another adult, that can care for the child. This form will provide us with the contact information for these situations.

  • Transportation Form
    The completion of this form will enable us to locate your child and plan our bus routes. This form should be completed for all new students and when your child pick-up or drop-off information changes.

  • Student Information Form
    This form assist the elementary staff in the placement of your child each year. The forms allows the parent to provide input into their child's specific personality and needs.

  • Free and Reduced Lunch Application
    This is the federal form required for the district to approve students for participation in the free and reduced lunch program.

  • Building Use Request
    This is the form to be used by outside groups who wish to make use of school district grounds and facilities. Please read the accompanying Board of Education policy prior to submitting this request.
