The Moravia High School nurse’s office will be providing school sports physicals between the hours of 8 a.m. and 12 p.m. on the following dates:
Wednesday, May 22, for high school students
Thursday, May 23, for middle school students
Wednesday, May 29, both high school and middle school students
Students planning to have physicals those days need to download, print and complete two documents, (the MCSD sports screening and New York state required school health exam forms), which are available under "Forms" below, and bring them to the school nurse on the day of the exam.
If you have difficulty accessing these documents, please contact Moravia High School nurse, Jennifer Read, at
jread@moraviaschool.org, who can provide hard copies.
